We are looking for the right people to work in our team centered homes! Care Haven Homes provides professional memory care in the warmth of a home. A successful, locally owned company, we operate 6 Alzheimer’s care homes in quiet Johnson County neighborhoods. Each houses a small community of 8 or fewer residents. Each home caters to a different level of cognitive & physical abilities, from Alzheimer’s early through late stages
This position provides quality and safe delivery of health care services in our homes under the direction of the Nursing Supervisor.
Essential Functions & Responsibilities
Provide assessments for potential incoming residents to determine eligible status for Care Haven Homes
Other duties as assigned
Minimum of a high school diploma or GED, 2-year associate’s degree and graduate of accredited school of nursing. Must have current Kansas State licensure as LPN. Two years professional nursing experience and current CPR certification. Must have skills and experience in wound care, phlebotomy, and cardiac assessment;
Extensive knowledge of commonly-used concepts, practices, and procedures within the nursing industry, specifically, a home-care setting and demonstrated comprehensive knowledge of office and facilities coordination and basic patient care knowledge;
Demonstrated organization, facilitation, communication and presentation skills;
Ability to prioritize workflow and organize diverse material and ability to handle multiple, competing and changing priorities; ability to perform effectively without supervision and within established time limits and ongoing deadlines;
Ability to interact and communicate effectively and professionally, and provide exceptional service, both internally and externally at all times.
Employee should be knowledgeable regarding Microsoft Office, have proficient computer skills, display adaptability in learning the use of company specific software and display comprehension of fundamental computer practices and database management;

While performing the duties of this position the employee is regularly required to sit and talk and hear. The employee is frequently required to use hands to finger, handle or feel and reach with hands and arms. The employee is occasionally required to stand, walk, and drive or ride in a motor vehicle. The employee must occasionally lift and/or move up to 25 lbs. Must have the ability to travel, sometimes with short notice.
